Insurance Accepted
Our office participates in most Managed Care health plans. In addition, we also accept all Medicare, Private Commercial Insurance, Workers Compensation and No-Fault Cases.

If you are unsure if SOA accepts your health plan please call our offices to confirm this information. In cases where our doctors do not participate in your plan, Seaport Orthopaedic Associates accepts Out of Network Benefits.

At the time of your visit you are responsible for your co-payment as determined by your individual insurance carrier. Seaport Orthopaedic Associates will submit your claims on your behalf for office visits, treatments and surgery.

Referrals
Before your initial visit, please check with your insurance carrier to see if you need a referral form from your primary care physician. Referrals can be sent to our office either electronically or in paper format.

Please be advised that some insurance companies take 48 hours to obtain a referral. If you DO need a referral have the referral made out to the doctor that you will be seeing at Seaport and bring it with you the day of your appointment.
